Until Livescan and Cardscan technologies are deployed and available for all requesters, hard copies of fingerprint cards will continue to be used. Instructions on where to mail hard copy fingerprint cards are included with e-QIP instructions. Blank fingerprint cards (FD 258) may be obtained from the following two locations:

DoD Contractors may request blank fingerprint cards from DISCO by either sending a email to occ.rqst.forms@dss.mil, faxing a request to 614-827-1523 or mailing a request to Defense Security Service.

All requests should reflect the company name, mailing address, cage code and number of fingerprint cards requested.

All other DoD organizations may request blank fingerprint cards from OPM by contacting 724-794-5612 x7481.
