The SF 86 requires that the Subject provide their Selective Service System registration number if they are born after December 31, 1959.  The registration number is NOT the same as the person's Social Security number.

Male applicants who do not know their Selective Service Registration number may obtain it by calling the Selective Service Data Management Center (it is at Great Lakes NAS, IL) at (847) 688-6888.  Individuals must use a touch-tone telephone and must provide their social security number and date of birth.
